Inside Athletic Training

Athletic trainers at the University of Minnesota Crookston collaborate with physicians and other healthcare professionals to provide care for student athletes. Athletic training focuses on the prevention, evaluation, and treatment of emergency, acute, and chronic medical conditions. Treatment of said injuries use a variety of evidence-based modalities and rehabilitation techniques. All athletic trainers at Crookston are nationally certified and licensed in the state of Minnesota.

Location and hours:

The athletic training room is located in Sports Center Room 153. It is attached to the men’s cross country/baseball locker room and the women’s cross country/softball/soccer/volleyball locker room. The athletic training room is open Monday – Friday, 9–5 or until the last practice of the day has concluded. Holidays and weekends are based around practice schedules.
For evaluation of non-emergent injuries, appointments are recommended. Please schedule with your team’s respective athletic trainer.

Pre-Participation Medical Requirements
 

Prior to participating in any team sanctioned practice, lift or competition all student athletes must complete the medical paperwork packet below. Please read the first page of the packet for information on what is included, what forms are required as incoming versus returning students athletes and how to get the completed packet to the athletic training staff.

If you had ADHD or ADD and are taking a stimulant medication as a management option, the below NCAA form needs to be completed as well. Your athletic trainer can provide further information on how to fill it out.

Medical Pre-Participation Paperwork

***Must be completed within 6 months from 1st day of practice***
*** Must be signed by a physician, physician assistant (PA) or nurse practitioner. A form signed by a physical therapist, chiropractor, or other health care professionals is NOT acceptable***
